What is Fenoldopam?

Fenoldopam is a dopamine D1 receptor agonist used as an intravenous medication primarily for the short-term management of severe hypertension, particularly in hospitalized patients. It works by directly stimulating dopamine receptors to induce vasodilation and reduce blood pressure. Fenoldopam has been studied in various clinical settings, including cardiac surgery, renal dialysis, and emergency departments, and is available in some countries under the brand name Corlopam. Its effects are generally rapid but may be short-lived due to receptor desensitization.

What is Fenoldopam used for?

Fenoldopam is an injectable medication that belongs to the class of dopamine agonists. It was approved by the US FDA in 1996 as a vasodilator, specifically indicated for the short-term management of hypertensive episodes (peaks) associated with autonomic instability in patients undergoing surgery for cesarean delivery. Fenoldopam acts as a selective DA1 receptor agonist, which results in peripheral vasodilation and an increase in renal blood flow.

The drug has been reported to decrease systemic vascular resistance without causing significant alterations in cardiac output or blood pressure levels, thereby allowing for controlled regulation of blood pressure during critical care situations such as acute hypertensive episodes. As a dopamine agonist, fenoldopam exerts its effects on the DA1 receptors present in blood vessels and kidneys, promoting vasodilation and diuresis respectively.

In clinical practice, fenoldopam is used off-label to manage various conditions related to cardiovascular instability, including shock and hypotension. However, it’s essential to use this medication with caution due to its potential for causing hypertensive crises or other adverse effects if not administered correctly.
